Coming Home After a Stroke: A Care and Rehabilitation Plan for the First 3 Months
What should families focus on when a loved one comes home after a stroke? Swallowing safety, positioning, exercise, communication and preventing another stroke.
Contents
A stroke happens when blood flow to part of the brain stops suddenly or a vessel bleeds. When hospital treatment ends, the longer journey begins: recovery and rehabilitation at home. The first three months are the most important window for regaining lost function.
Preparing the home
- Set up the bed on the ground floor near a toilet if possible.
- Allow access from both sides of the bed, especially the affected side.
- Arrange a hospital bed, pressure mattress, wheelchair or walker in advance.
- Get the medicine list, exercise advice and follow-up dates in writing at discharge.
| Area | What to do | Why |
|---|---|---|
| Swallowing safety | Texture-modified food, upright eating | Prevents aspiration pneumonia |
| Positioning | Support the affected arm and leg with pillows | Reduces shoulder pain and spasticity |
| Movement | Daily exercises, physiotherapy | Strength and joint mobility |
| Communication | Speech therapy, picture cards | Helps self-expression |
| Mood | Watch for depression | Motivation and recovery |
| Risk control | Blood pressure, sugar, medicine adherence | Prevents another stroke |
Swallowing problems
Coughing during meals, a "wet" voice or food pooling in the mouth suggest dysphagia. Feed the patient sitting upright, keep them upright for 30 minutes afterwards, give small bites, thicken liquids if advised and ask for a speech and swallowing assessment.
Protecting the affected side
Never lift the patient by pulling the weak arm; support it on a table or pillow when seated. Approaching and placing items on the affected side encourages attention to it.
Rehabilitation at home
Regular exercise supports neuroplasticity. With a physiotherapist: rolling, sitting and standing practice; balance and gait training; fine motor tasks; stretching to reduce spasticity.
Preventing another stroke
Keep blood pressure and sugar on target, take blood thinners and cholesterol medicines without gaps, stop smoking and reduce salt. New speech problems, facial drooping or limb weakness mean call 112 immediately.
We combine carers, nurses, physiotherapists and speech therapists in one care plan across Yalova, with discharge assessments in Bursa, Istanbul and Kocaeli–Gebze.
