Companionship for an Older Person Admitted with a Hip Fracture: From Surgery to Walking
How the hospital process goes after a hip fracture in older age: waiting for surgery, early mobilisation, preventing delirium and pressure sores, discharge and home rehabilitation.
Hip fractures in older people usually follow a simple fall at home and change life suddenly; osteoporotic bones can break with minor impact. It is a serious health event, but with prompt surgery, early movement and good care many people walk again. A companion's support can be decisive.
| Stage | Description |
|---|---|
| Emergency assessment | X-ray, blood tests, heart and lung evaluation |
| Before surgery | Pain control, fluids, optimising other conditions; surgery as soon as possible for most |
| Surgery | Screws, plates, nails or joint replacement depending on fracture |
| Early post-op | Pain management, clot prevention, early mobilisation |
| Discharge and rehab | Physiotherapy at home or in a rehab centre |
Before surgery
Give staff the full medicine list (especially blood thinners), report changes in alertness, reassure the patient, ensure fasting is followed and help with repositioning to protect the back and heels.
After surgery
- Early mobilisation: often within the first day, with a physiotherapist and nurse, reducing pneumonia, clots, pressure sores and muscle loss.
- Delirium prevention: glasses and hearing aids, familiar surroundings, day–night routine and reporting sudden confusion.
- Pressure sore prevention: float the heels on a pillow and reposition regularly.
- Nutrition and fluids: protein and energy for healing and strength; help at meals and report intake.
- Clot prevention: prescribed injections and ankle exercises.
| Topic | Advice |
|---|---|
| Physiotherapy | Regular home sessions for walking, balance and strength |
| Home layout | High chair, raised toilet seat, grab bars, non-slip floors |
| Walking aid | Walker, later a cane on advice |
| Bone health | Osteoporosis treatment, vitamin D and calcium review |
| Fall prevention | Home safety, medication review, balance exercise |
A person who has had a hip fracture is at high risk of another; osteoporosis treatment and fall prevention matter as much as rehabilitation.
We support older hip-fracture patients with experienced companions in hospitals across Yalova and plan continuous recovery at home with physiotherapy, nursing (blood-thinner injections, wound care, stitch removal), carers and equipment, with services in Bursa, Istanbul and Kocaeli–Gebze.
