Crown (Vertex) Hair Transplants: Whorl Direction, Graft Planning and Timeline

Why crown transplants are different: the circular growth pattern, graft needs, prioritising front versus crown, slower results and protective treatments.

The crown (vertex) is a common site of male-pattern loss. Unlike the front, it is hard to see in the mirror and is often noticed in photos or by others. Crown transplants have their own technical and planning needs.

Feature Explanation
Circular direction Hair spirals out from a central whorl
Large area The crown can widen and need many grafts
Progressive loss Loss often continues for years
Slower results Development may lag behind the front
Visibility Hard for you to see, easy for others

Recreating the whorl

For a natural crown, grafts must follow your natural whorl direction; otherwise hair may stand the wrong way. DHI's graft-by-graft angle control is an advantage here.

Prioritising front or crown

With limited donor and loss in both areas, prioritise: the front usually comes first, as it frames the face; the crown must be planned considering future widening. Very dense crown work in young people with fast loss can create a ring-like look if new gaps form around it later.

Crown area Approximate grafts*
Mild thinning 800–1,500 for densification
Moderate 1,500–2,500
Large 2,500+

Timeline

Growth and thickening at the crown can be slower; judge the final result at 12–18 months.

Growth after transplant
Allow 12–18 months to judge the crown's final result.

Protective treatment

As loss continues long-term here, medical treatment, PRP, mesotherapy and regular follow-up on medical advice help protect surrounding hair; mild crown thinning sometimes improves with these alone.

For large crowns with limited donor, scalp micropigmentation can complement a transplant to enhance the look of density.

At our Istanbul clinic, crown work is planned around whorl direction and likely progression, combined with protective treatments and pigmentation when needed; clients from Yalova, Bursa, Kocaeli and Gebze can share crown photos for a preliminary assessment.

Frequently asked questions

Why are crown results slower?
Circular direction, area size and circulation differences; allow 12–18 months.
Front or crown first?
With limited donor, usually the front, with the crown planned around progression.
