Hiding Donor Thinning: Scalp Micropigmentation After Overharvesting
How SMP camouflages thinning and white dots in the donor area after an overharvested transplant: planning and expectations.
Taking more grafts than the donor can provide (overharvesting) leaves the back of the head sparse, patchy or "moth-eaten", and FUE points can show as white dots with short haircuts. Even with a good frontal result, this makes short styles difficult. SMP is an effective way to camouflage donor thinning.
| Cause | Explanation |
|---|---|
| Overharvesting | Exceeding capacity |
| Uneven extraction | Clustering in the same areas |
| Outside the safe zone | Follicles that may fall later |
| Multiple sessions | Cumulative strain |
| Large punch size | More visible dots |
How SMP helps
Micro-dots matching the density and tone of existing follicles are placed in sparse areas, white dots and thinning blend with surrounding hair, and the back of the head looks more uniform.
Planning: the donor must be fully healed; assess hair colour and short-cut appearance; start light with tone; usually 2–3 sessions.
| Hair length | Effect |
|---|---|
| Very short (clipper 1–3) | Most visible camouflage |
| Short–medium | Thinning much less visible |
| Long | Less need, as hair covers the donor |
Alternatives and complements: balanced grafting into the donor using alternative donors such as the beard (where suitable), a slightly longer haircut, or a combined plan.
Expectations: SMP markedly reduces visible thinning but does not add hair; it works best with short hair; touch-ups may be needed.
The best solution is prevention: a first transplant that respects donor capacity with balanced extraction from the safe zone.
At our Istanbul clinic, donor thinning and white dots from previous transplants are assessed, with SMP alone or combined with balanced grafting; clients from Yalova, Bursa, Kocaeli and Gebze can share short-hair photos of the back of the head.
