Why Is My Beard Patchy? Genetics, Hormones, Age and Options to Consider Before a Transplant
Why beards grow sparsely: genetics, the roles of testosterone and DHT, beard development with age, alopecia areata and options to consider before a transplant.
"Why is my beard patchy?" Beard growth varies hugely between men, usually due to genetic and hormonal factors. Understanding the cause helps choose the right solution before a transplant.
| Cause | Explanation |
|---|---|
| Genetics | Density and pattern are largely inherited; relatives give clues |
| Hormone sensitivity | Beard growth depends on testosterone and DHT, but follicle sensitivity, not hormone level, is usually decisive |
| Age | Growth continues in many men into the mid- or late 20s |
| Ethnicity | Density differs between populations |
| Alopecia areata | Round hairless patches |
| Scars | Accidents, surgery, burns or acne |
| Hormonal disease | Rarely |
In most men with normal hormone levels, a sparse beard reflects genetic differences in follicle response, so "hormone therapy" is generally not appropriate and must never be used without medical advice.
Growth with age
Puberty brings fine hair at the moustache and chin; at 18–25 the cheeks and sides fill in; many men reach final density after 25. Young men may wait and watch before considering a transplant.
Alopecia areata
Round, well-defined hairless patches may be alopecia areata, an autoimmune condition that should be assessed by a dermatologist; transplants are usually not advised while active, and it may improve spontaneously or with treatment.
| Option | Explanation |
|---|---|
| Waiting and monitoring | For young men |
| Topical treatments on medical advice | May increase density; effects vary and may fade when stopped |
| Beard styling | The right style can disguise sparse areas |
| Beard pigmentation | Mimics hair to create density |
| Beard transplant | Permanent solution for suitable candidates |
Topical treatments and medicines must be used with medical advice and follow-up.
A transplant is right when beard development is largely complete with clear gaps remaining, there are permanent causes such as scars, a permanent solution is wanted and donor supply is adequate.
At our Istanbul clinic, beard structure, age and the cause of sparseness are assessed, with alternatives suggested instead of or before a transplant when appropriate; clients from Yalova, Bursa, Kocaeli and Gebze can send photos for a preliminary assessment.
