Repairing a Failed Hair Transplant (Revision): What Can Be Done with DHI?

Unnatural hairline, sparse results, wrong angles or an overharvested donor: options for repairing a failed hair transplant and the role of DHI.

Repairing a Failed Hair Transplant (Revision): What Can Be Done with DHI?

A transplant does not always deliver the expected result. An unnatural hairline, poor density, hair growing the wrong way or a visibly thinned donor can harm confidence. Revision surgery aims to improve previous results and needs careful planning.

Problem Appearance
Unnatural hairline Too straight, too low or sharply angled
Large grafts at the front Pluggy tufts instead of fine hairs
Wrong angle or direction Hair standing up or growing backwards
Low density Visible scalp
Poor survival Many grafts failed to grow
Overharvested donor Patchy, "moth-eaten" back of the head
Island effect Isolated transplant area as loss progresses
Scar Linear scar from an old strip (FUT) procedure

Before revision

Usually wait at least 12 months to see the final result, reassess donor capacity, identify the type of problem (design, density, angle, donor) and agree realistic goals: not every problem can be eliminated, but clear improvement is often possible.

Options

  • Softening the hairline with fine single grafts placed in front via DHI.
  • Increasing density without damaging existing grafts, helped by DHI's angle control.
  • Removing misplaced grafts and relocating them where appropriate.
  • Donor camouflage through balanced small-scale grafting or scalp micropigmentation.
  • Scar camouflage with grafting or pigmentation over old FUT scars.
Challenge Explanation
Limited donor Much may have been used already
Scar tissue Blood supply may be altered
Existing grafts Must not be damaged
Expectations Limits must be explained honestly

Asking about the doctor's role, design approach, donor planning and realistic expectations before a first procedure greatly reduces the need for revision.

At our Istanbul clinic, clients unhappy with a previous transplant receive a detailed assessment, and DHI, graft removal and scalp pigmentation are planned individually once donor capacity, problem type and goals are clear; clients from Yalova, Bursa, Kocaeli and Gebze can share previous information and photos at consultation.

Frequently asked questions
