The lymphatic system collects excess fluid, proteins and waste from tissues and returns them to the bloodstream, and it is part of the immune system. When it is damaged or insufficient, fluid accumulates and persistent swelling develops: lymphoedema. It may not disappear completely, but early diagnosis and regular treatment keep it well controlled.

Type Cause Examples
Primary Congenital lymphatic development problems Can appear in childhood, adolescence or adulthood
Secondary Later damage to the lymphatic system Lymph node removal in cancer surgery, radiotherapy, infection, trauma, obesity

A common cause of secondary lymphoedema is breast cancer treatment involving axillary node removal or radiotherapy; gynaecological, prostate and melanoma treatments can cause leg lymphoedema.

Symptoms

Heaviness and fullness, rings, watches or shoes feeling tight, tight swollen skin, pitting when pressed (early), later skin thickening, reduced joint movement and recurrent skin infections (cellulitis).

Stage Feature
Stage 0 (latent) No visible swelling, impaired lymph transport, possible heaviness
Stage 1 Soft pitting swelling that reduces with elevation
Stage 2 Swelling no longer reduces with elevation; tissue firms
Stage 3 Marked enlargement, thickened skin, folds and skin changes

The earlier treatment starts, the better. Report new swelling, heaviness or tightness after cancer treatment.

Leg swelling has other causes: heart failure, kidney and liver disease, varicose veins, DVT and some medicines. Sudden, painful, red swelling of one leg needs urgent assessment for DVT.

Treatment: complete decongestive therapy

  1. Manual lymphatic drainage: gentle, specialised massage
  2. Compression bandaging with multi-layer short-stretch bandages
  3. Compression garments to maintain results
  4. Exercise to activate the muscle pump
  5. Skin care to prevent infection

Daily life

Protect the limb from cuts, scratches and insect bites, moisturise daily, avoid saunas and sunburn, avoid tight jewellery and clothing, maintain a healthy weight, and see a doctor promptly for redness, warmth and fever.

Our trained physiotherapists provide manual lymphatic drainage, bandaging and exercise programmes at home across Yalova, with plans in Bursa, Istanbul and Kocaeli–Gebze.
