Frozen Shoulder Rehabilitation at Home: Stages and Exercise Programme
Why frozen shoulder happens, the stages it goes through and which exercises to do at home: pain management, range of motion work and recovery.
If shoulder pain and stiffness keep increasing when combing your hair, putting on a jacket or reaching a high shelf, you may have a frozen shoulder (adhesive capsulitis). The joint capsule thickens and contracts. It is more common between 40 and 60, in women and in people with diabetes. It can last a long time, but most people recover with proper rehabilitation.
Risk factors: diabetes, thyroid disease, prolonged immobility after injury or surgery, an unused arm after stroke, heart disease and Parkinson's.
| Stage | Approx. duration | Main feature | Rehab focus |
|---|---|---|---|
| Freezing (painful) | 2–9 months | Increasing pain, night pain, reduced movement | Pain control, gentle movement |
| Frozen | 4–12 months | Less pain, marked stiffness | Stretching, range of motion |
| Thawing | 6 months–2 years | Gradual return of movement | Strengthening, function |
Durations vary widely. Diagnosis needs a medical examination to rule out rotator cuff tears or arthritis.
In the painful stage
Avoid forcing movements but do not immobilise the shoulder; medicines or injections may be prescribed; warmth before and cold after exercise can help; a pillow under the arm can ease night pain.
Basic home exercises (your physiotherapist sets your programme)
- Pendulum: lean on a table, let the arm hang and swing it in small circles for 1–2 minutes, several times a day.
- Finger walk: walk fingers up a wall to the highest pain-free point; 10 repeats.
- Towel stretch: hold a towel behind your back and pull it up with the good arm.
- Cross-body stretch: bring the arm across the chest, supported at the elbow, for 15–30 seconds.
- Stick external rotation: lying down, elbows bent at 90°, push a stick with the good arm to rotate the painful arm outward.
Rules
Feel a gentle stretch, not sharp pain; short sessions 3–4 times a day work better; avoid sudden forceful movements; be patient.
In diabetes, frozen shoulder may last longer and recur; good glucose control helps, and blood sugar may rise for a few days after steroid injections.
We provide home shoulder rehabilitation across Yalova, coordinated with medical assessment, with sessions in Bursa, Istanbul and Kocaeli–Gebze.
